Therapy:

$185.00 Initial Intake Session

$150.00 per 50-minute individual session
$180.00 per 50-minute family/couples therapy session

Medication Management:

$250.00 Initial Intake Session

$200.00 per 30-minute follow-up appointment

The rates above are the out of pocket rates without the usage of insurances we accept.

    We accept the following insurances:

  • Anthem (commercial policies only, we are not in network with Anthem Healthkeepers Plus Medicaid)

  • BlueCloss BlueShield

  • Carefirst

  • BCBS Federal Employee Program

  • United Healthcare/Optum

  • Cigna

If you have a different health insurance than the ones listed above, we are considered an out-of-network provider. We do not accept Aetna, Kaiser, Medicaid, or Medicare at this time. We will be happy to supply you with documentation which you can use to request out of network reimbursement. To find out how our services would be reimbursed by your insurance company, you will need to contact them directly. We recommend asking your insurance company the following questions:

 

  • Can I be reimbursed for mental health therapy sessions/medication management by an out-of-network provider?

  • Is each session reimbursed at an equal rate, or am I reimbursed more or less over time?

  • Is there a limit on how many sessions I can be reimbursed for over a certain period of time or over the life of my insurance plan?

  • Does my provider have to provide me with a diagnosis in order for me to submit my claim for reimbursement?

 

 

Payment:

We accept cash, check, debit/credit cards (Visa, Mastercard, Discover, & AmEx), and HSA/FSA.

 

Checks can be made payable to:

"Cloud Break Therapy, LLC". There is a $30 fee for a returned check.

 

Cancellations:

Be advised, however, there is a $120.00 cancellation fee if you cancel without a 24 hour notice. Feel free to talk to your provider; We understand illness, jobs, child care and weather cause us all difficulty at times, but please be sensitive and aware that your provider's time is as equally important as yours.

 

Emergencies:

We do check our voice mails and emails regularly, even on the weekends. If you experience a mental health emergency, please call 911, or go to your local hospital or emergency clinic and give them your provider's number. All releases of information can be faxed to (703) 289-0385 or emailed directly to your provider.
